"I hereby authorize my child(ren) to participate in "Hollywood Photo Shoot" at the California State Fair on Monday, August 25th, 2008. I understand that my child(ren) will have their photograph taken by Lisa Welge and the photograph will be distributed to me by the email address provided below. I understand that all photographs taken remain property of Lisa Welge, and may be used for promotional purposes, including, but not limited to, websites, professional blogs, and online and print portfolios. I am free to print or make copies of the photos I am emailed. I understand that I am not to use these photographs for my own commercial or promotional use, unless a separate agreement is made between Lisa Welge and myself."
